isolved ENROLLMENT INSTRUCTIONS – use this cheat sheet if you’re logging in to the system for the first time to enroll in your 401(k) plan
DISTRIBUTION AT TERMINATION – use this form if you’ve separated service and would like to move your account elsewhere and/or take a cash distribution.
Special Tax Notice2 –please reference this information when taking a distribution from your retirement plan.
Beneficiary Designation Form – This form is required ONLY if you’re married and designating someone OTHER than your spouse as your primary beneficiary. If your spouse is your 100% primary beneficiary, your elections can be made online – no form required.
HARDSHIP WITHDRAWAL REQUEST If you’re an active employee and need to access your account, you may qualify for a hardship distribution for one of the following reasons: 1. To purchase a primary residence, 2. To prevent eviction or foreclosure, 3. For post secondary education expenses for you, spouse, or dependent, 4. For funeral expenses, 5. For repair to a primary residence due to natural disaster. You will be required to provide documentation along with your hardship application.
